  Lots of variations. Most clients who present a risk of violence are in jail or prison and most jails have a way to meet privately but under the observation of jailers. Others just take their chances. A friends of mine got stabbed by his own client in the middle of a trial recently. After getting medical care, he finished the trial but the client was in restraints during the rest of the trial.
